This Is How It Works

Individual Sales Representatives:

1. Each organization representative will receive a catalog page and Card Order Form.

2. Fill in one line on the Card Order Form, for each sale.

3. After all sales are made, total the number of sets for each card style sold.

4. Turn in the Card Order Form to the organizer.

Fundraiser Organizer:

1. Fill out the Tally Sheet, one line for each representative.

2. Total the number of card sets sold.

3. Multiply by $10 for Total Sales.

4. Apply a 40% discount.

5. Add a Service Fee of $5 if Total Sales (before discount) is less than $170.

6. This is the Amount Due.

7. Submit Tally Sheet and Payment to Pet Star Corporation using option A or B:

* Organizations within Florida, please submit a copy of your Tax Exempt Certificate

A – Supply credit card information and signature on Tally Sheet.

Fax to Pet Star at: 305-255-3366

B – Make a check payable to Pet Star Corporation for the Amount Due.
